By signing this waiver form, I agree to release What It Takes Training/Shackleford Basketball Camps and Clinics, its coaches employees and volunteers from all damages that may occur while my child is participating in the What It Takes Training/Shackleford Basketball Camps and Clinics. I will hold harmless for all injuries incurred. I understand that I am responsible for all medical treatment including COVID-19 resulting from camp/clinic injuries. By signing this form, I also verify that my child is covered by medical insurance. Further, I hereby agree that What It Takes Training/Shackleford Basketball Camps and Clinics or it representatives, may video tape, photograph and voice record the herein named applicant for media, marketing, or promotional purposes related to his/her participation in What It Takes Training Camps and Clinics/ Shackleford Basketball. This may include online, photo display, and or other promotional opportunities.
